  • Additional Info: As an expansion of Summit Health's Primary Care Team, we are seeking a Family Medicine Nurse Practioner to care for our growing population. This position will not host a traditional panel but will truly act as an extension of our Primary Care Providers.
    Responsibilities will include:
    - Seeing Same-day patients
    - Follow up visits
    - Patient education
    - Assisting with In-Basket Messages
    - Being the key link to delivering quality care supporting our open access model
    Qualifications:
    - Current OR State APN License
    - Minimum 1-years' experience in Primary Care
