Requirements:



  • Rates and availability must be submitted at the time of name clear - Required

  • Board Certified: REQUIRED

  • Minimum of 2 years of experience: REQUIRED

  • OH License: REQUIRED, no exceptions

  • Proof of COVID Vaccination Status: REQUIRED

  • BLS/ACLS: Current at the time of submission

  • Clean Background, No Malpractice: REQUIRED, no exceptions


Certification Requirements:
