Maple City Health Care Center

- Hours:Flexible Monday - Friday schedule; 4 day work week available

- Average Patients seen: 18-22 per day
- Call Schedule: Shared call
- Loan Repayment
- Compensation: Salary is based on education, experience, responsibility, and language proficiency with additional compensation for bilingual (Spanish/English) providers

- Health insurance; 100% of premiums paid for employee, 50% for spouse/dependents
- Will cover 80% of out of pocket costs up to a limit of $4,000 for spouses on their own plans

- 3 weeks PTO year 1; 4 weeks PTO starting year 2 - Additional Info: The Family Physician is a member of the cross-disciplinary team that includes nurse practitioners, medical assistants, nurses, behavioral health, and other support staff.
Responsibilities:
- Offer family practice primary care to the patient population at MCHCC
- Provide clinical leadership for the care team
- Meet with the care team twice daily to review upcoming appointments and individual patient needs
- Serve as a collaborating physician on collaborative practice agreements for nurse practitioners and midwives
Qualifications:
- DEA and Controlled Substance Registration Certification required
- Bilingual Spanish preferred
- Trauma informed care approach preferred
- Motivational interviewing training preferred
