  - $180,000 - $205,000 Based on Experience
  - Sign-On and Relocation package

  DETAILS:
  - Home site with willingness to travel within 30 miles (four sites)
  - Assess physical and psycho-social health status through medical history and physical examination
  - Care team acts as "coaches" who help their patients get healthy, stay healthy, and get the care and services that are right for them.
  QUALIFICATIONS:
  - NY License REQUIRED
  - 3 yrs experience in Community Health PREFERRED
  - Hospital Privileges PREFERRED
