Geriatrician needed to work in a 325 bed Medical Center.  Full time opportunity. This position is located only minutes from Philadelphia. Work for a large multi-specialty group in Southern New ; The group offers an excellent salary with incentive bonuses. They offer a full compensation package.

 



  • Licensed to practice medicine in New Jersey

  • In good standing with the Medicare and Medicaid programs

  • Board certified/eligible in Internal Medicine or Family Practice

  • Experience in the care of chronically ill and frail seniors

  • Certificate of added qualifications in geriatrics - preferred

  • BCLS Certified 

  • Minimum of 4 years of experience in Medical Practice, including at least one year of geriatric practice required

  • Previous experience working with an interdisciplinary, care management and care planning, and performing comprehensive geriatric assessments


Key responsibilities:


  1. Assumes responsibility for the provision and documentation of medical care of participants assigned to the interdisciplinary team


a. Performs comprehensive history and physical on new referrals and participants

b. Performs and documents initial medical intake on prospective participants

c. Performs and documents periodic face to history and physical on established 

    participants as scheduled

d. Completes initial assessment and semi-annual reassessment face to face without 

    delegating to Nurse Practitioner (if any)

;Admits participants to the hospital:

f.  Provides primary care (attending) responsibilities for inpatient care management

;Judiciously refers participants to medical specialist as indicated, and assists in the 

    monitoring of specialist services and ancillary products or services

h. Manages all care of participants in the nursing homes and assisted living 

    facilities/programs:

i.  Provides regular visits to participants in nursing homes and assisted living 

    facilities/programs:

j.  Performs and documents telephone contact with nursing home staff as required;

k. Admits nursing home participants to the hospital when necessary.

l.  Documents changes in the participant’s condition and details care provided by 

   completing all required documentation and ensuring compliance to our standards and 

   policies including required hospital and nursing home documentation and medical 

   oversight.

with Medical Director and other members of IDT to assure 24 hour 

    coverage of care

n. Serves as collaborating physician for our Nurse Practitioners as requested by

    Medical Director
