Requirements:



  • Board Certification: Required

  • License: Active ND License, or IMLC (Interstate Medical Licensure Compact) - Required

  • Certifications: BLS, DEA - Required

  • Availability: Must state provider's specific dates of availability for listed open dates

  • Clean Malpractice/Background: Highly Preferred

  • CHI/Dignity Facility History: Must disclose if provider has ever worked at any other CHI/Dignity facility

  • COVID Vaccination: Required
