  • Additional Info: Job Description:
    - Will assist in the daily medical care, admissions, and discharges of patients on the Summit Health hospitalist service. The APP will work in close supervision with the physician staff of the Summit Health hospitalist department. The APP will provide support to ensure achieving value- based quality performance metrics such as, quality evidenced based care, right post acute setting, patient safety, decreased length of stay, reduce readmission rates, etc. This position will require several different care settings: predominantly skilled nursing facilities and home visits.
    Job details:
    - Identifying chief complaints/issues and directing medical care for patients in an inpatient setting
    - Providing detailed, focused medical assessments and management of patients and transition them back to ambulatory settings from the hospital
    - Review acute medical illness, assess expected recovery, evolve care plan, and educate patients and families
