Looking for a Nurse Practitioner responsible for the assessment, medical case management, diagnosis, treatment, follow-up and referral of clinic patients seen at the hospital's Rural Health ;
The Nurse Practitioner provider plans patient care using skills specific to his/her individual training and experience and works under established ;Such care may include but not be limited to:
1. Taking patient history – data base gathering.
2. Physical Assessments – episodic and ongoing.
3. Ordering needed lab, x-ray and other testing specific to the patient problem (diagnosis) and in accordance with established protocol.
4. Initiates treatment which is appropriate to the patient problems, diagnoses, and specific to the individual needs of the patient…cultural, ethnic, sociological, religious, emotional and physical needs are taken into consideration in planning for care.
5. The Mid-level provider is responsible for establishing a plan of care which will assist the patient to move toward better health and healing.
6. Education, counseling and appropriate referral will be carried out to assure that patients’ ongoing needs are met as well as episodic problems cared for.
7. Ongoing case management can include discussion of patient care problems with supervising physician and referral to a physician or specialist when needed.
8. Furnishing of appropriate medications and treatment according to established protocol-including respiratory therapy, physical or occupation therapy, speech therapy or other appropriate ;
Minimum of 4 10-hour days a week, preferably 5 8-hour days a week. Approximately 20 patients per day.
