  DETAILS:
  - Scheduled Post-Discharge visits
  - Provides primary care diagnosis and treatment in a family practice clinical setting to patients recently discharged from an inpatient or emergency department setting.
  - Counsels and educates patients regarding treatment and refers to ancillary and support services.
  - Will rotate between all of our primary care clinics (Coeur d'Alene, Post Falls, Rathdrum, and Kellogg)
  QUALIFICATIONS:
  - Minimum 2-4 years of experience directly related to the duties and responsibilities specified
  - State of Idaho RN Licensure, DEA, and Idaho Controlled Substance or ability to receive licensure required.
  - BLS certification or must obtain within 90 days of hire
