Job Summary:


AdvantageCare Physicians (ACPNY), one of the largest primary and specialty care practices in New York City, is seeking an experienced Family Nurse Practitioner to join our Expanded Access service line. This program focuses on providing high-quality patient care by offering a full scope of primary care services for adult and pediatric patients.

Ideal Candidate Qualities:



  • 3-5 years of experience as a Family Nurse Practitioner in primary care.

  • Proficient in treating acute and chronic conditions in primary care, womens health, and pediatrics.


Expanded Access Differentiators:



  • Ensures acute needs align with patients' ongoing health needs and relationships with other ACPNY providers.

  • Works closely with existing primary care physicians, specialists, and nurses to coordinate and close gaps in care.


Schedule:



  • Monday-Thursday: 9:30 am - 5:30 pm

  • Friday: 8:00 am - 5:00 pm


Responsibilities:



  • See same-day appointments during extended hours.

  • Obtain and document patient history based on the chief complaint or reason for the visit.

  • Conduct relevant physical exams and evaluate patients, documenting findings.

  • Diagnose and treat diseases, managing chronic illnesses.

  • Prescribe medication in accordance with New York State and federal law.

  • Order appropriate laboratory, radiological, and special examinations or tests; interpret results.

  • Provide preventative health services, including education and counseling.

  • Collaborate with other clinicians and staff to ensure continuity of patient care.

  • Close patient gaps in care, such as HEDIS, hospital follow-up, and goal-setting.

  • Participate in quality initiatives under the direction of the Express Care Service Line Chair and site Medical Office Medical Director.


Qualifications:



  • Nurse Practitioner: Master of Science degree in Nursing (MSN).

  • Graduate of a Nurse Practitioner education program registered by the New York State Education Department.

  • Certification as a Nurse Practitioner from a New York State approved accrediting body.

  • 3 years minimum experience, capable of providing care without physician supervision.

  • Recent experience treating both adult and pediatric patients.

  • Recent experience providing basic gynecological care (, womens exams, Pap smears).

  • Current, unrestricted New York State license or eligibility.

  • Active Drug Enforcement Agency (DEA) registration or eligibility.

  • Active Medicare/Medicaid Enrollment or eligibility.

  • Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification required (both strongly preferred).

  • Ability to suture and splint a plus.

  • Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.
