  • At least 5 years of Primary Care clinic experience

Qualifications:

• Licensure:
  
  
  
  • Arizona state license required (Compact license accepted)
  
  
  • DEA license required
  
  
  • BLS certification required
  
  
  
  


• Board Certification: Family Nurse Practitioner (FNP)
