  • Key Responsibilities:



    • Conduct comprehensive patient evaluations, including health history, physical examinations, and diagnostic testing.

    • Develop and implement individualized treatment plans for a variety of acute and chronic conditions.

    • Provide preventative care services, including immunizations, screenings, and health education.

    • Manage chronic conditions such as diabetes, hypertension, and asthma.

    • Perform basic procedures such as injections, IUD placement, and minor wound care.

    • Collaborate effectively with other healthcare professionals to ensure seamless patient care.

    • Document patient care accurately and efficiently within the EPIC electronic medical records system.




  • Qualifications for Success:



    • Current national certification as a Family Nurse Practitioner (FNP) is mandatory.

    • Valid and active Washington State nurse practitioner license is required.

    • Current DEA registration for Washington State is mandatory.

    • Minimum of 2 years of experience as an FNP with recent patient care experience (within the past year) is required.

    • Experience working in a community health center setting is preferred, but a strong desire to serve vulnerable populations is a plus.

    • Fluency in a second language is a plus, but not required.

    • Excellent communication, interpersonal skills, and a patient-centered approach are crucial.

    • The ability to work independently while collaborating effectively within a team environment is essential.
