    JOB DETAILS:
    -Passion and excitement for a unique and challenging primary healthcare service
    -Clinically adept, with the ability to communicate and educate a diverse patient population
    -Experienced and comfortable treating chronic conditions such as COPD, hypertension, diabetes, etc
    -Demonstrated patience to develop a progress plan and assist patients with their goals.
    -Give access to specialty care services, to alleviate the need for escalation to hospital services
    -Possesses the ability to work autonomously, providing support to both patients and support staff alike
    ADDITIONAL REQUIREMENTS:
    -BE/BC, State licensed: Will consider applicants from the DMV area
    -1 year experience required, 2 years preferred in a primary care setting in within a clinical environment, managing chronic conditions.
    -Candidates must have demonstrated the ability to provide assessments, provide thorough screening, and assist with lifetime support
