Responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes) and process design (leading initiatives to facilitate practice-based and organizational change).
3-5 years nursing experience in diabetes management and education
8:00am - 4:30pm; Monday - Friday
