• Additional Info: CHCB envisions a model primary care integrated delivery system that promotes prevention and wellness while ensuring that all people have access to quality, comprehensive and coordinated healthcare.
  - In collaboration with Behavioral Health staff, provides assessments, evaluations, consultations, examinations, differential diagnosis, and treatment planning to a select group of patients.
  - Provides clinical direction to the clinical staff including Social Workers, Case Managers, RNs, LPNs, and Medical Assistants.
  - Functions and contributes as part of a multidisciplinary team.
  - Current opening at our home site in Buffalo, NY. CHCB operates 4 sites within 30 miles.
  Qualifications:
  - NY license
  - DEA registration number to prescribe controlled substances
  - 5 years of experience in an outpatient setting is preferred but not required
  - Experience with eClinical Works EMR preferred
