  -Initiates care for those requiring mental health services throughout
  Bayless care continuum.
  -Provide health/behavioral health assessment (including differential diagnoses,
  prognosis), treatment recommendations and treatment planning for clients across
  program types.
  -Provide medication management for clients on medication and intermittent
  reassessment (as needed)
  -Supports Mission, Vision, and Values of Agape Family Health and understands the outpatient setting of an FQHC
  -The provider will be required to float between several facility sites
  Qualifications:
  -Must be certified as a PMHNP
  - 3 to 5 years clinical experience preferred
  -Must have a valid DEA Number and unrestricted license in FL.
  - Must be proficient in EMRs
  - Will float to King St, Dunn Ave, University, N. Pearl St, Blanding, and Merrill Rd locations.
