  • Additional Info: The NP provides primary health care to the frail elderly population, focusing on disease prevention, wellness, management of minor, chronic, acute illness, palliative, and end of life care. The NP provides holistic care, taking into account the needs and strengths of the whole person. The NP works in close collaboration with the entire interdisciplinary team to establish realistic goals through a participant /patient plan of care.
    The NP will work in our PACE department. PACE is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
    Qualifications:
    - One or more years of experience as a nurse practitioner in the setting of geriatrics preferred.
    - Kansas license and passed certification exam for chosen specialty required
    - Graduation form an accredited nurse practitioner program
    - Must already be or willing to obtain Medicare/Medicaid certification
