Five Rivers Health Centers

  • Full Time
  • Hours:M- F 8 AM to 5 PM(40 hour workweek)- 4 day work weeks available (32 weekly hours)

  • Average Patients seen: 18 patients daily
  • Call Schedule: 2 weeks of on-call once a Quarter ( low-volume, phone only)
  • Compensation: Negotiable Compensation
    - Organization Incentive - Up to $2000 incentive per quarter based on department budget, total patients, and quality measures.

  • Additional Info: - Hiring for the WHY
    - This opening is for their new location opening in January 2022. The provider will start at their Miami Valley facility and then will be moved to the Edgemont Campus when it opens.
    - EMR - Epic
    - This facility offers a "Centering Parenting Program" - a group visit for well-child checks. The sessions are longer than a normal visit and you meet with a group of patients that are all due for their WCC. This allows the provider more time to educate parents/patients and allows the parents/patients to learn from each other.
