• Additional Info: The University Physicians Charlottesville outpatient clinic at Fontaine seeks an experienced Advanced Practice Provider to join a well-established outpatient clinic in Charlottesville, VA. Providing team-based adult primary care, the selected LIP will be responsible for the following:
  - Medical assessment based on patient history, symptoms and physical findings
  - Development of evidence-based treatment plans
  - Ordering or performing relevant diagnostic tests
  - Patient education on self-management of acute or chronic illnesses
  - Patient-facing care will be provided for eight sessions per week, distributed among direct, collaborative, and triage patient care. The remaining two sessions per week will be allotted to pre- and post-clinical care.
