  • Hours:Monday - Friday 8 am - 4 pm | Available via phone 7 am - 8 am and 4 pm - 5 pm

  • Call Schedule: 3rd call every 10 weeks | RN First Call, NP Second Call

  • Compensation: $225,000 - $275,000, based on experience

    You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community with a strong focus on the participant's goals as well as excellent geriatric care management.
    QUALIFICATIONS
    - At least one year of experience working with a geriatric population
    - Current MA license, DEA prescription privileges, and CPR Certification
    Preferred
    - BC in Geriatric Medicine
    - Suboxone Waiver
    - At least one year of experience working with low-income and diverse cultural groups
    Ability to
    - Quickly recognize situations/problems before becoming acute, implement effective solutions, and understand consequences
    - Convey information clearly and succinctly to team members
