Required Procedures:
Conventional Radiotherapy
3D Radiotherapy
External Beam Radiation - 3D-CRT
External Beam Radiation - IGRT
External Beam Radiation - IMRT
Orders Tests, Orders Images, Prescribes
Board Certified - REQUIRED
Active OH License & DEA - Highly preferred; will review providers needing to obtain license
BLS Certified - REQUIRED
COVID Vaccinated - Noted at time of name clear
